s4c irb2400 m98 cold start error

Hi colleagues

While doing restart the controller, I have got a problem.
Everytime I insert the disk4, it ask me to put disk 4 again and I hit any key then it restarts and timer goes forever.
Eventually, I had to turn the power off and back on.
then I need to do cold start procedure again.

I have replaced the disk 4 to new one and tried different revision number like 06, 55.
But still the same problem.

Please give some hints.
